Kindergarten, 1st and 2nd Grade

DTap/DTP/Td

5 doses

4 acceptable if 4th dose given after 4th birthday(See footnote on card if <3 injections at enrollment)

Polio

4 doses

3 acceptable if 3rd dose given after 4th birthday

Hepatitis B

3 doses

MMR

2 doses

1st dose must be given >12 months of age for child

Varicella

2 doses (at least 28 days apart)

- or- History of disease documented on immunization record from doctor’s office or lab confirmation

Grades 3 through 5

DTap/DTP/Td

5 doses

4 acceptable if 4th dose given after 4th birthday(See footnote on card if <3 injections at enrollment)

Polio

4 doses

3 acceptable if 3rd dose given after 4th birthday

Hepatitis B

Note: If parent wants an exemption for personal or religious beliefs, the parent must sign and date the back of the Immunization Card and note which shots they don’t want their child to have.

If there is a medical exception, a doctor must sign and date the back of the Immunization Card.

Enrolled in a Colorado school Prior to July 2000

Note: There may be unusual cases where odd exceptions apply because of the footnotes on the bottom of an Immunization Card. If it is unclear whether the student is/isn’t compliant, page your nurse.
