Non-Permission FormsAttention: If you would like to sign a "Parent Non-Permission Form" for print, video and electronic media and internet and electronic mail access, please stop by our front office or call, 488-6600. We will give you a stamped form that you may sign if you wish. PSD school activties and events are occasionally photographed or video taped, or students are interviewed by PSD staff and/or representatives of the news media. If you any reason you do NOT wish to have your child(ren) interviewed, photographed or videotaped, request the form, fill it in and return it to the school within 5 days from the date indicated on the bottom of the form.
