Records Forms

There are two types of documents for the Records Center Forms - printable forms and forms that can be filled in and/or signed electronically. 

 

The "Printable" Forms may be printed and then sent to us by fax, email, or US mail.

The "Electronic" forms may be filled in electronically, then emailed to psdrecords@psdschools.org, sent US Mail or faxed to 970-490-3003.

Adobe Reader Users:  The electronic form may be filled in in all areas excluding the electronic signature.  A signature must be manually written on the form, then faxed, emailed or mailed back to us. 

Adobe Acrobat Users:  After you have opened the form, you can electronically sign the form.  Left click on the 'signature' line.  If you have not set up an electronic signature before, click on the button 'to create a new digital signature now'.  Choose 'Windows Certificate Store" - the most common and easiest choice.  Enter your name and email address in the fields.  Click on 'finish', then 'sign' your form.  You will need to save the form (for example to your desktop) before you attach it to an email back to us.
