Records may be requested by the parent/legal guardian or eligible student. An 'eligible' student is defined by federal law as a student who has reached the age of 18 or is attending a post secondary educational institution. Records will not be released to parents of eligible students without the student's signed consent. If the eligible student has been claimed as a dependent on the parent or guardian's federal tax return for the previous year, a Declaration of Dependency Status Form must be completed and submitted along with the request and any applicable fees. Requests can be mailed, faxed, emailed, or submitted in person.
An official and unofficial transcript are the same document; however, the official transcript has a raised seal and a signature of a school official and is sealed in an envelope for authentication and shouldn't be opened or copied by the student. Colleges and Government agencies generally require an official transcript. Official transcripts cannot be emailed or faxed. An unofficial transcript is a simply a copy of a transcript. To request transcripts of a student not currently enrolled in Poudre School District, download and complete the Request for Student Transcripts/Records form. (Click here to see the fee schedule.) We do not respond to Education Verification requests over the phone. Please note the request can be mailed, emailed, faxed or presented in person. Additional contact information and directions can be found on the Records and Transcripts main page. Unless the parent/guardian is otherwise advised, a student's special education records will be destroyed according to Colorado State Archives guidelines five years after the student moves to another school district, graduates, or otherwise stops receiving special education services from Poudre School District. In order to receive notices from the District during this five year period, the parent/guardian must keep their address on file with the District current. The parent/guardian or student may request to review or obtain a copy of the records at the PSD Records Center.
For students/parents/guardians:
We recommend that anyone seeking copies of their student's special education file review the file first to avoid unnecessary copying and to save everyone time. To schedule an appointment to review a file, contact the Records Center at 970-490-3142 or psdrecords@psdschools.org. You will need to bring a picture ID with you at the time of your appointment.
To obtain a copy of a special education file, we must receive a complete and legible request that is signed by the parent/legal guardian or eligible student. Outside agencies must include a signed release from the student over the age of 18 or from the parent/guardian if under the age of 18, along with your request.
The PSD Records Center does not manage academic records for active PSD students. Records for active PSD students must be requested through the school the student is currently attending.
