Benefits Overview

Overview of Options

Benefits-eligible employees have the choice to opt into any of the following for themselves, their spouses, and/or eligible dependents:

  • Medical insurance
  • Dental insurance
  • Vision insurance
  • Voluntary group term life insurance
  • Flexible spending accounts (including medical reimbursement and dependent care)
  • Tax-deferred savings plans (including 401(k), 403(b), and 457)
  • Basic life insurance/accidental death & dismemberment
  • Short- and long-term disability

A wide array of wellnesslifestyle healthmental healthurgent care services, and other benefits are also available.

Enrollment process

The process for benefits-eligible new employees will guide you through selecting and signing up for benefits. Please follow the steps below to view your benefits options and complete your online enrollment.

Note: If you enroll in benefits after the 15th of the month in which your benefits are effective, you will see a double deduction in premiums on your following month paycheck.

Step one:

Find the eligibility notice e-mailed to your PSD email explaining when your benefits will be effective (this will be the first day of the calendar month following the date you meet the eligibility requirements) and which benefits you will be eligible for.

Step three:

  1. Complete the enrollment forms attached to the eligibility email you received.

  2. Return completed forms to PSDBenefits@psdschools.org or fax to 970-490-3624 within 31 days of your noted effective date.

  3. If you enroll your spouse or eligible dependent children on your medical, dental, or vision plans, you will be required to submit Proof of Relationship documents to Benefits Services before your enrollment is approved. A list of acceptable Proof of Relationship documents is attached to the Eligibility Notice that was e-mailed to you by Benefits Services.  Email the necessary documents to PSDbenefits@psdschools.org

  • Note: Your completed enrollment must be received within 31 days of your benefits effective date.

Medical Cards will be mailed to your home address from Simplified Benefits Administrators within 10-14 business days from the completion of your enrollment process. Replacement cards must be requested from Simplified Benefits Administrators at 1-866-644-7873 or 1-800-207-1018.

Dental: Delta Dental does not send physical cards in the mail. Please visit deltadentalco.com and register to print out a card; you will not receive cards in the mail. You must download the mobile app AND/OR create an account online (using your SSN as the member ID). 

Prescription Cards will be mailed to your home address from OptumRx. within 15 days of your enrollment completion. Replacement cards must be requested from OptumRx at 800-880-1188.

The Vision Service Plan does not issue cards.

Step four:

  • If interested, enroll online in the optional Legal Service Plan offered by ARAG. 

    *Your online enrollment for this plan must be submitted within 31 days of your benefits effective date. Otherwise, you are not eligible to enroll in these optional benefits until the next open enrollment period.

  • If interested, complete the optional Voya's Life AD&D Enrollment Form to elect accidental death and dismemberment coverage for you and any eligible family members. Send the completed form to the PSD Benefits Services Department. 

  • Complete the Voya's Beneficiary Designation Form for the $50,000 basic life insurance policy provide to you by PSD. Send completed form to the PSD Benefits Services Department.

Benefits Administration Contacts

Health Benefits: PSD's health benefits are self-funded and administered by Simplified Benefits Administrators. For questions regarding a claim status, deductibles, FSA questions, and/or changing your PCP, please contact Simplified Benefits Administrators at 1-866-644-7873 or 1-800-207-1018. Access a list of providers on Select Health's website. 

Dental Benefits: Dental benefits are administered by Delta Dental. For any questions regarding a claim status or your annual out-of-pocket maximum, visit this page. To find an in-network provider, please also visit deltadentalco.comYou will not receive membership cards in the mail; you must download the mobile app AND/OR create an account online (using your SSN as the member ID). 

Prescription Benefits: Prescription benefits are offered through OptumRx. For questions about prescription costs and/or annual out-of-pocket maximums, contact OptumRx at 1-800-880-1188.

Vision Benefits: Vision benefits are offered through Vision Service Plan. For questions on how the coverage works, co-payments, claim status, or reimbursements, contact Vision Service Plan at 1-800-877-7195. Access a list of providers on VSP's website.

Mental Health Benefits: PSD's mental health benefits are self-funded and administered by Employee Assistance Services (EAS). If you have questions regarding EAS mental health benefits, community mental health network providers, referral authorizations, copay/coinsurance, deductibles, mental health out-of-pocket maximums, etc., please contact EAS at (970) 488-4925 or visit their website.
