Medicaid Outreach and Enrollment Services

Poudre School District's SHS Medicaid program uses a portion of the Health First Colorado reimbursement funds to increase outreach to uninsured and underinsured families. We support our mission to "educate every child every day" by personally helping families fill out the applications for Health First Colorado, CHP+, and SNAP.

How we help

• Removing barriers that prevent families from completing new applications and renewal paperwork
• Explaining complicated paperwork sent from the state regarding coverage
• Checking status of completed applications
• Connecting to resources when coverage is not possible through Medicaid or CHP+
