Incident Report

 

What it is:

Students, parents and staff can report experiences that negatively impact their education based on race, gender, culture, religious belief, socio-economic status and sexual orientation.

Examples:

  • Discrimination based on culture or ethnicity 
  • Bullying 
  • Racial and homophobic slander 
  • Anti-Semitism    

 

How it works:

  • Reports are responded to within three business days (this is not monitored 24/7).
  • Reports by students and parents are routed to the PSD Director of Language, Culture and Equity. 
  • Reports by staff are routed to the PSD Executive Director of Human Resources. 
  • Reports may be anonymous or identifiable. 
  • Tip and messages are only accepted through the PSD Incident Reporting platform.

 

Process after a report is made:

  • Student-to-student report: The report is sent to the Director of Language, Culture and Equity to review within three business days.

  • Student-to-staff report: The report is sent to the Director of Language, Culture and Equity to review within three business days.

  • Staff-to-staff report: The report is sent to the Executive Director of Human Resources to review within three business days.
