The Records Center has a print or electronic form that you may submit by email, fax, mail or in person. All request for records forms must have a signature to process.
Official Transcript: $3.00 each
Unofficial Transcript: No Charge
The Records Center accepts cash, checks or money orders made payable to Poudre School District, and credit/debit cards. (Please note for all payments made by credit or debit card, there is a convenience fee of approximately 3.35%.)
Requests will not be processed without both a release form and payment.
Electronic forms: Please note, you may have to download the form in order to e-sign it.

Electronic forms must be digitally signed (do not simply type your name in the signature field) which includes a date/time stamp.
Adobe Acrobat Reader Users: The electronic form may be filled in all areas excluding the electronic signature. An electronic or digital signature cannot be obtained with Adobe Reader. You will need to print the request at this step then sign the form manually or use an acceptable form of electronic signature (see above). The form can then be faxed, emailed or sent US Mail. Please note, we must have a signature to process your request.
Adobe Acrobat Professional Users: To electronically sign - left click on the 'signature' line. Click on the button 'to create a new digital signature now'. Choose 'Windows Certificate Store." Enter your name and email address in the fields. Click on 'finish', then 'sign' your form. You will need to save the form before you attach it to an email to the Records Department or you may print and fax or send it via US Mail. Please note, you may have to download the form in order to e-sign it. We must have a signature to process your request.
